To participate in WellspringHeart, patients must meet the screening criteria. A medical history is taken and baseline in-office tests are performed. A Registered Dietician will discuss fundamental principles of the diet.
- Results of lab values
- Risk factors
- Blood pressure/heart rate
- Body mass index (BMI)
- Measurements
- Goals are set
Those who meet the initial screening criteria will be referred for a CT Angiography at Silverton Hospital; the results of which will be discussed with the patient by the program Medical Director. Should the results of the CT Angiography and screenings suggest the need for WellspringHeart intervention, the Nurse Case Manager will discuss the program in detail with the participant and schedule enrollment. Eligible participants include:
- Patients with a history of Coronary Artery Disease (CAD), MI, stent, angioplasty, bypass surgery, CABG, diabetes; or
- Individuals at high risk for CAD, male over 45 or female over 55 with two of the following:
- Elevated lipids (cholesterol)
- Hypertension
- Those medicated for any of the above
- Family history of heart disease
- Obesity
- History of smoking
